[ ] This Filler Club Membership auto-renews monthly until cancelled. By enrolling you expressly consent to auto-renewal and authorizing Nowak Aesthetics to charge your payment method every month at the current rate, which is $199. There is a minimum commitment of 12 months. Monthly membership payments shall be made in advance by direct debit from the member's designated credit/debit account, which will be kept on file by Nowak Aesthetics. Nowak Aesthetics reserves the right to increase the monthly price of the membership, however, will not do so without advanced notice of at least 30 days. You will receive an email acknowledging your enrollment that includes these terms.

[ ] Enrollment Fee: I acknowledge a non-refundable enrollment fee of $199.00. First Filler will be 90 days after enrollment. Enrollment fee can be waived if you have had a filler treatment with us in the last 30 days.

[ ] Cancellation: To cancel your membership, call (619) 420-1840, email concierge@drnowak.com, or cancel online at https://store.drnowak.com/customer/account. If the membership is cancelled prior to the 12-month minimum commitment, a $100 cancellation fee will be applied for each of the four treatments that are not redeemed during the initial 12 months.

[ ] I acknowledge that services are earned and must be rendered every 3 months. It is the Member’s responsibility to ensure that services are rendered timely. Membership payments will continue monthly regardless of services scheduled, and unused membership payments will not be eligible for a refund. All treatments and benefits from this membership are non-transferable.

Temporary Hold: To pause your membership, you must be in good standing with membership payments. Nowak Aesthetics reserves the right to cancel the membership if it is determined that un-pausing the membership is unlikely. Nowak Aesthetics will provide written notice in the form of an email prior to any cancellation.

Declined payments: Members who are in a failed payment status for more than 30 days will be eligible for membership cancellation. It is the member's responsibility to maintain a valid credit card on file for payment of the monthly fee. In the event of a failed payment, Nowak Aesthetics will attempt to contact the member to obtain a new form of payment.

Membership discounts cannot be combined with monthly promotions and specials. To receive discounted pricing online during your membership, you must be logged into your account on the store and follow the instructions during checkout.

Nowak Aesthetics reserves the right to cancel or change this membership at any time without notice. Nowak Aesthetics reserves the right to refuse membership to anyone at our discretion. Nowak Aesthetics reserves the right to review memberships periodically and will provide at least 30 days’ written notice of any changes, including increases in the membership fee or changes to the date of automatic withdrawal.

Treatments will consist of 1 Filler chosen by the provider based on individual needs and concerns. Any additional Fillers added on the day of treatment will be charged an additional fee.
