Enrollment Fee: $0.00. To qualify for a Balance Club, a completed BHRT package is required.

Monthly membership payment: $159.00 recurring monthly with a minimum 12-month commitment. Monthly membership payments shall be made in advance by direct debit from the member's designated credit/debit account, which will be kept on file by Nowak Aesthetics.
Cancellation: A $300 early cancellation fee will apply unless proof of moving 50 miles outside the Chula Vista area. To cancel your membership, a 30-day written notice is required.
Temporary Hold: To pause your membership, you must be in good standing with membership payments and can pause for a maximum of 4 months with an automatic re-activation.
Declined payments: Members who are in a failed payment status for more than 30 days will result in a membership cancellation. It is the member's responsibility to maintain a valid credit card on file for payment of the monthly fee.

Terms: Services are earned and must be rendered every 3 months otherwise treatment will be forfeited, and payments will be non-refundable.

Treatments will consist of a BHRT pellet insertion treatment and DIM SGS+ supplements are included. One Annual Lab review is also included. Any additional BHRT services will be charged regular pricing with membership discounts applied.

All treatments and benefits from this membership are non-transferable.

Membership discounts cannot be combined with monthly promotions and specials. To receive discounted pricing online during your membership, you must be logged into your account on the store and follow the instructions during checkout.

We reserve the right to cancel or change this membership at any time without notice. We reserve the right to refuse membership to anyone at our discretion.

We reserve the right to review memberships periodically and will provide at least 30 days’ written notice of any changes, including increases in the membership fee or changes to the date of automatic withdrawal.
